AVN of Metatersal Head
Most commonly affects 2nd metatarsal head, known as Freiberg disease. Rarely can affect 3rd and 4th and can be bilateral. Freiberg disease is characterized as osteochondrosis of the second metatarsal head. It is the fourth most common form of primary osteochondrosis.

- Thought to be related to a disruption in the blood supply (osteochondrosis ) due to microtrauma or overloading. Commonly seen in adolescent athletic female population. More common in people with long 2nd metatarsals.
You will feel pain at fore foot difficulty in activity specially running and jogging.
You should visit a orthopaedic or a specialist foot and ankle surgeon if you develop pain at 2nd metatarsal area with difficulty in 2nd toe movement.
Your doctor will conduct a proper clinical examination which will include checking the movement and deformity of 2nd toe. You may ask to get an x-ray of your feet in standing position. If indicated a MRI may also be required.
To start with conservative treatment in the form of NSAID, cold pack is helpful. In case of significant symptom immobilisation may require. Local infiltration of steroid is also practised, but there is difference of opinion. Surgical treatment is indicated if there is gross destruction of joint and limitation of movement. Osteotomy of the metatarsal neck is done with excision of necrotic fragment and repositioning of healthy articular surface is done to preserve the joint function.
